Valley Hope of Grapevine

The patient and any other individuals have the right to express opinions, recommendations and grievances regarding treatment, safety, and/or fee schedule without fear of prejudice or penalty.
 
When possible, individuals are encouraged to submit any concerns to facility leadership. It is the responsibility of the Executive Director or his/her designee to address those concerns and have a written response within 7 calendar days of receiving the grievance.
 
If a concern cannot be resolved at the facility level, individuals are encouraged to submit their concern to the Valley Hope home office.

In Texas facilities, in addition to the above, writing materials, postage and access to a telephone will be provided as needed.  Patients who are unable to read or write will be given any needed assistance.  The patient or family may file a grievance with:
